How to Fill in the Online Request Form

If you are using the online request form, the following information is required to action your request:

• You will need to log in using your UC username to use the request forms. Your details will then be inserted automatically.
• Please give a date specifying when the request is no longer required.
• Type in the title of the item (full) and the author.
• If you want a particular edition, please state the edition or year of publication.
• If you require an article from a journal, please give the page numbers of the article, the article title and author and the journal title and volume number.

Check the location of the item you require in the library catalogue

And choose the appropriate location on request form
